29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  a.) Main street, Sparta NJ   An employee was performing bricklaying activities from a pallet that laid on the forks of the all terrain forklift truck to reach elevated levels, there-by being exposed to an fall hazard of approximately 15ft from the supported scaffold platform to the ground below, on or about 09/25/2017.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a.) Main street, Sparta NJ   Employees performing bricklaying activities from a pitched roof in order to reach higher levels without the use of a personal fall arrest system, exposing them to a fall hazard of approximately 15ft to the ground below, on or about 09/25/2017
