On or about September 25, 2017 and prior to that time located at 11332 and 11334 Tamarco Dr. Blue Ash Oh. there was no frequent and regular inspection program being maintained in use by a competent person to ensure the job site, materials and equipment such as maintaining systems for the structural stability of the metal decking during dismantling, fall protection systems and an access extension ladder being maintained secured from displacement or barricading systems were being maintained and utilized in a safe condition.
