29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying exists:    a.  On or about August 10, 2017, at the above addressed jobsite, an inspection was not conducted by a competent person capable of correcting hazards such as falls, which could result in employee injuries.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a.  On or about August 10, 2017, at the above addressed jobsite, an employee conducting siding operations from a ladder jack scaffold was not trained to recognize the hazards associated with scaffolding.  The employee was thereby exposed to the hazards associated with falls.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a.  On or about August 10, 2017, at the above addressed jobsite, an employee performing siding operations from a ladder jack scaffold, twelve (12) feet four (4) inches above the ground, did not utilize a personal fall arrest system, thereby exposing the employee to falls.
