29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees.  a) Jobsite: Inside excavation - Two underground gas lines were not protected or supported as necessary to safeguard employees working inside the excavation installing a new sewer line. Employees were using shovels and a 8.5k lb Case Mini Excavator in proximity to the gas lines inside a trench measuring approximately 19 ft. in length, ranging from 4 ft. to 3 ft. in width, and ranging from 7 ft. to 6 ft. in depth.   Observed on or about 9/25/17.

Employees were conducting plumbing operations inside an excavation measuring approximately 19 ft. in length, ranging from 4 ft. to 3 ft. in width, and ranging from 7 ft. to 6 ft. in depth. Protection by placing and keeping such materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary was not provided.  a) Jobsite: The employer exposed his employees to falling object hazards by not placing excavated material with loose rock at least 2 feet away from the edge of the trench. Employees were installing new sewer lines inside the trench measuring approximately 19 ft. in length, ranging from 4 ft. to 3 ft. in width, and ranging from 7 ft. to 6 ft. in depth.  Observed on or about 9/25/17.
