29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about 26 September 2017, and at times prior thereto, employees performing tool changes for a MetalMizer Vertical Band Saw with two energy sources did so utilizing a General Lockout/Tagout Procedure instead of a specific procedure for the saw, exposing the employees to an amputation hazard.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about 26 September 2017, and at times prior thereto, the MetalMizer Vertical Band Saw was not locked out or tagged out when a blade would be changed, exposing the employee to an amputation hazard due to being caught in or struck by the moving blade.

29 CFR 1910.303(g)(1)(i)(A): Working space about electric equipment rated 600 volts, nominal, or less was not provided with the minimal depth in the direction of access to live parts as indicated in Table S-1. On or about 26 September 2017, and at times prior thereto, an electrical disconnect switch for a MIG welding tool did not have at least 36 inches clearance in front and around the switch, and prevented quick and ready access, exposing employees using the tool to an electrical hazard of approximately 240 volts.
