29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, shall be provided to assist employees in mounting and dismounting the ladder.  On September 26, 2017 at 4061 Hudson Hill Drive in Hobart, employees were using an extension ladder to gain access to the a single story roof.  The ladder did not extend 3ft. above the roof edge and was not secured to prevent displacement.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  On September 26, 2017 at 4061 Hudson Hill Drive in Hobart, employees were using a extension ladder to  access the roof on a residential house that was not positioned at a 4:1 angle.  The ladder was setup and used with a 2:1 flat angle.  The horizontal distance between the ladder's top support 10.5 ft. and the foot base was 5 ft.

On or about September 26, 2017 at 4061 Hudson Hill Drive in Hobart, employees were exposed to fall hazards without receiving adequate training on the use of personal fall arrest equipment.  Employees were not utilizing any fall protection equipment when installing roof shingles at heights greater than 6 feet.

Employees using an extension ladder to access/egress a roof surface with improper angle, positioned with top blocked not allowing extension of side rails 3 ft. above roof surface, and ladder not secured.
