29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide. On September 26, 2017 at 828 Casimir Street in Pulaski, an employee was walking/working from a 4 in. wide board at 8.5 ft. above surface level. The 2 in. x 4 in. x 14 ft. boards were placed on the bottom chords of trusses as a catwalk.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. On September 26, 2017 at 828 Casimir Street in Pulaski, an employee was accessing/egressing an elevated surface with a 30 in. break in elevation. A 6 ft. stepladder was used to access/egress a work area 8.5 ft. above surface level.
