29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about September 25, 2017 and times prior thereto, the employer did not ensure that employees replacing sheet metal on the auger housing at a cotton gin was protected by guardrail system, safety net system or personal fall arrest system. Employees were exposed to a fall hazards in excess of 15 feet.

29 CFR 1926.1412(f)(2): 29 CFR 1926.1412(f)(2): The equipment was not inspected by a qualified person at least every 12 months in accordance with paragraphs (f)(2)(i) -- (xxi) of this section. On or about September 26, 2017 and times prior thereto, the employer owned and used a mobile crane and did not ensure that it was inspected at least every 12 months in accordance with paragraph (f)(2)(i) -- (xxi) of this section. Employees were exposed to the hazard of being struck by falling load.
