29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At a residential construction site, an employee working from the second level landing area above the stairway without fall protection was exposed to an unprotected fall hazard of approximately 10 to 12 feet to the concrete below.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. At a residential construction site, employees working from the second level landing area where guardrails were installed, the guardrail did not have a mid-rail installed between the top edge of the guardrail system and the walking/working surface exposing employees to fall hazards of approximately 10 to 12 feet to the concrete floor below.

At a residential construction site, the employer did not ensure the guardrail system overlooking the front door area was capable of withstanding a force of at least 200 pounds applied in any outward or downward direction exposing an employee to a fall hazard of approximately 10 to 12 feet to the ground below.
