On or about September 27, 2017:   The employees drawing blood, collecting urine, and administering injections while caring for patients at Snapfinger Woods Family Practice Group Clinic are exposed to bloodborne pathogens.  The employer did not provide bloodborne pathogen training to these employees.

On or about September 27, 2017:  The employees drawing blood, collecting urine, and administering injections while caring for patients at Snapfinger Woods Family Practice Group Clinic are exposed to bloodborne pathogens.  The employer did not maintain medical records of these employees with information to include but not limited to their hepatitis B vaccination status.

On or about September 27, 2017:  Employees working at the facility located at 5071 Snapfinger Woods Drive are exposed to trip hazards and smoke and fire hazards where the fire extinguishers were not identified.
