29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: JOBSITE: The employer did not ensure safe and acceptable means were used to determine the estimated location of an underground electrical power line. On or about September 27, 2017, the bucket of a Volvo track hoe, Model ECR88D struck an underground electrical power line while digging, and it caused an electrical flash fire. An employee sustained burn injuries from the incident.
