29 CFR 1926.451(g)(1)(ii): Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system:    (a) KLM Framing LLC at 3063 Brighton Blvd, Denver, CO 80216: On and before September 26, 2017 the exposing employer did not ensure that each employee engaged in window installation from a Spider Rolling Gantry with 5x5 Outrigger on the eighth floor of a high-rise apartment building at a height of approximately 55 feet was protected by a guard rail system. An employee hung from the side and guardrail of a two-point suspension scaffold, while wearing an independent personal fall arrest system, to install a window, where the scaffold did not provide access.  During these activities the employee was not protected by a guardrail system and the employee was potentially exposed to hazards associated with fall arrest and suspension trauma.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  (a) KLM Framing LLC at 3063 Brighton Blvd, Denver, CO 80216: On and before September 26, 2017 the exposing employer did not ensure that each employee engaged in window installation from a Spider Rolling Gantry with 5x5 Outrigger on the eighth floor of a high-rise apartment building at a height of approximately 55 feet was trained to recognize the hazards associated with a two-point suspension scaffold and to understand the procedures to control or minimize those hazards. An employee hung from the side and guardrail of a two-point suspension scaffold, while wearing an independent fall arrest system, to install a window, where the scaffold did not provide access. This condition potentially exposed the employee to hazards associated with fall arrest and suspension trauma.     Abatement Note: The training shall include the following areas, as applicable: 1. The nature of any electrical hazards, fall hazards and falling object hazards in the work area; 2. The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used; 3. The proper use of the scaffold, and the proper handling of materials on the scaffold; 4. The maximum intended load and the load-carrying capacities of the scaffolds used; and 5. Any other pertinent requirements of this subpart.
