29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces: A) Near Loading Rack - On or about September 28, 2017, horizontal pipes containing, but not limited to, natural gas, naphtha, and liquid asphalt were installed at ground level in front of Tank #21. Employees needed to step over these pipes in order to access the heater controls on the tank and the asphalt transfer valves on the pump station near the loading rack. The employees were exposed to a tripping hazard.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: A) Worksite - On or about September 28, 2017, employees who mixed corrosive chemicals during the manufacture of asphalt emulsions and added asphalt additives while loading trucks were supplied with safety glasses instead of safety goggles. Safety glasses would not protect the employees from liquid chemicals entering the eyes.
