29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Mechanical Shop - The DoAll Horizontal Band Saw was not equipped with a guard covering the unused portion of the rotating saw blade. An employee reported using the saw during the period between September 1, 2017 and October 3, 2017.  (b) Biomedical - The Ryobi bench top drill press was not equipped with an automatically adjusting guard over the rotating drill. An employee reported using the drill press during the period between July 1, 2017 and October 3, 2017.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:  (a) In the Transportation Maintenance Shop - The Balador pedestal mounted grinder along the North wall had the adjustable peripheral protecting members (tongue guards) removed from the unit, leaving it unable to be adjusted to the decreasing diameter of the wheels. During the period between July and October, 2017, an employee used the grinder in the course of his duties.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  (a) Biomedical - Employees engaged in testing energized electrical circuits (for voltage and/or amperage) are not trained in and familiar with safety-related work practices. During the period of July 1, 2017 and October 2, 2017,  an employee used a multimeter to test incoming electrical voltage on energized 480 volt electrical circuits.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  (a) Biomedical Department - Employees who's duties required them to perform electrical testing on energized circuits were not provided with electrical arc/flash resistant protection for the face and body. During the period of July 1, 2017 and October 2, 2017, an employee used a multimeter to test incoming electrical voltage on energized 480 volt electrical circuits.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
