29 CFR 1910.28(b)(3)(i): Holes. The employer did not ensure that each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower lever:    (a)  BUILDING 3, STATION 37 - Employees worked in the upstairs portion of station 37 to perform set-up and feeding operations for the milling that takes place in the lower part of the station, and the employees were exposed to an approximate 13 foot fall to the floor below, on or about September 28, 2017.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
