29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) Construction site north wall: Employees were exposed to fall hazards approximately 12 ft to concrete floor below when tubular welded frame scaffold was not fully planked while employees were working with formwork. Condition was observed on or about September 28, 2017.

Construction site north wall; a) Employees engaged in formwork activities and using a tubular welded frame scaffold, that was approximately 12 ft high, were not protected from falling to concrete floor below by a fall arrest or guard rail system. Condition was observed on or about September 28, 2017.
