29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area. On or about 10/02/2017 at the 122 Prospect Hill Road Site, employees were working from an aerial lift and were not trained on the fall hazards of working from an aerial lift using personal fall arrest systems. The employees had not been trained on the fall distance clearance requirements.
