29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease) by a minimum distance of 20 feet. Employees in the maintenance shop were exposed to fire and explosion hazards in that Oxygen cylinder(s) were stored approximate 5 feet from 4 gasoline cans with one being a full 5 gallon can, located near the southeast area of the shop.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose; and each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: At the north elevator employees were exposed to explosion and fire hazards in that the railcar counter secured in the area known as the main floor was missing a cover exposing energized live wires where grain dust is common.
