29 CFR 1915.87(d)(1): The employer did not provide and maintain adequate first aid supplies that were readily accessible to each worksite: a) Date of Violation: On or about 08-28-2017, located at Colonna's Shipyard, Inc., 400 East Indian River Road. Norfolk, VA 23523, located in the Inside Machine Shop, an employees left little finger was caught between a POREBA lathe chuck and the manual locking mechanism which resulted in a fingertip amputation. There were no first aid supplies readily available and the employee used a soiled rag to control bleeding. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, and rotating parts: a) Date of Violation: On or about 08-28-2017, located at Colonna's Shipyard, Inc., 400 East Indian River Road. Norfolk, VA 23523, located in the "Old IMS Building", an employee's left little finger was caught between a POREBA lathe chuck and the manual locking mechanism which resulted in a fingertip amputation. No Guarding was installed or provided at or near the nip point. Colonna's Shipyard, Inc., was previously cited for the same violation of this Occupational Safety and Health Standard OSH ACT of 1970, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1021407, citation number 1, item number 1 and was affirmed as a final order on 03-11-15, with respect to a workplace located at Colonna's Shipyard, Inc., Inside Machine Shop, 400 East Indian River Road. Norfolk, VA 23523. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov: Instance #1: Date of Violation: On or about 05-04-2017, located at Colonna's Shipyard, Inc., 400 East Indian River Road. Norfolk, VA 23523, located in Building #7, an employees left thumb was caught between a flange-face and a mechanical sanding jib causing a partial traumatic metacarpophalangeal amputation. The employer did not report the amputation to OSHA. Instance #2: Date of Violation: On or about 08-28-2017, located at Colonna's Shipyard, Inc., 400 East Indian River Road. Norfolk, VA 23523, located in the "Old IMS Building", an employees left little finger was caught between a POREBA lathe chuck and the manual locking mechanism which resulted in a fingertip amputation. The employer did not report the amputation to OSHA. Abatement certification required within 10 days after abatement date.
