29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels. On or about 27 September 2017 and at times prior thereto, employees were working in an environment where the 8-hour time-weighted average for noise was at or above 85 decibels without receiving annual audiograms, exposing the employees to standard threshold shifts and hearing loss.
