29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: (a) On or about October 3, 2017, employees were exposed to shock and burn hazards where knockouts on a breaker panel in the east side of the main shop were missing. (b) On or about October 3, 2017, employees were exposed to shock and burn hazards where knockouts an outlet box connected to the above breaker box were missing. (c) On or about October 3, 2017, employees were exposed to shock and burn hazards where a knockout on a junction box attached to the above breaker panel was missing. (d) On or about October 3, 2017, employees were exposed to shock and burn hazards where a knockout was missing on a 250V outlet box used to plug in the arc welding machines.
