29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews. On or about July 13, 2017, at the North end of the Sanctuary, above the Communion section, the Scaffolding company failed to install a guardrail at the end of the walking-working platform.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. On or about July 13, 2017 inside the Sanctuary, the employer failed to conduct daily inspections on the scaffold by a competent person.
