29 CFR 1910.178(n)(6): The forklift operator driver did not keep a clear view of the path of travel. b) On or about and prior to October 2, 2017, forklift operators would push multiple pallets of packaging material with the pallets of packaging material that was on the forks of the forklift which obstructed the view of the path in the direction they were travelling.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about October 2, 2017, employees that worked on the Brite Stacker #11 can palletizer were exposed to struck-by hazards from the movement of the stacking arm. The Brite Stacker #11 did not have the pressure mat installed between the operator and the stacking arm.
