Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 10/02/2017, an employee from Fred Holder was installing roofing materials on the roof of a residential building located on lot 690 on Mule Creek Drive in Wentzville, MO. He was not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The owner and the employee were observed on a 7/12 sloped roof approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.
