The employer did not maintain warehouse floors free from excessive ice accumulation, resulting in slip hazards. Condition noted on or about 8/30/2017.

Exit route outside of the rear freezer emergency exit door was blocked by two bikes and some cardboard. Condition noted on or about 8/30/2017.
