29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe: (a) R & I Construction, Inc. on or about September 27, 2017, at the site located on Ohio 53, quarter mile north of Ohio 103, McCutchenville, OH: The employer did not implement a rigging inspection program. A double leg synthetic web sling utilized to hoist components of a mobile crane was not inspected for deficiencies prior to use. Multiple snags and cuts were present contributing to sling failure under load, exposing employees to struck-by injuries.

29 CFR 1926.251(e)(8)(iii): Synthetic web sling(s) were not immediatly removed from service when snags, punctures, tears, or cuts were present: (a) R & I Construction, Inc. on or about September 27, 2017, at the site located on Ohio 53, quarter mile north of Ohio 103, McCutchenville, OH: The employer did not ensure a double leg synthetic web sling used to hoist components of a mobile crane was removed from service when several snags and cuts were present in the material. While utilizing the damaged 10 ft. long nylon sling in this condition, employees were exposed to struck-by injuries.
