29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition: a) On or about 10/4/2017, in the pool mechanical room, where work was being done to fix a pipe, metal support material, hoses, electrical cords, and 2 x 4 planks were across the floor making it difficult to safely walk and work in the area causing slip and trip hazards. b) On or about 10/4/2017, in the pool mechanical room, where work was being done to fix a pipe, there was standing water causing a slip hazard.

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: a) On or about 10/4/2017, in the pool area where employees apply pool chemicals such as but not limited to Aqua Balance Calhypo Shock and Aqua Balance Muriatic Acid, PPE is not provided by the employer for employees to use while applying these chemicals.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) On or about 10/4/2017, in the laundry room where employees use detergents such as but not limited to Tide Detergent, the emergency eyewash is not accessible in accordance with ANSI/ISEA Z358.1 6.4.2. The emergency eyewash is blocked by a door and by the slop sink which does not allow the employee to easily access the emergency eyewash. b) On or about 10/4/2017, in the pool mechanical room the eye wash is blocked by hoses and a mop bucket. The employer did not ensure the emergency eyewash and shower were free from obstruction in an area where injurious corrosive pool chemicals are applied.
