Right Pointe, LLC, Emergency eye wash/shower station-South wall of production area, on or about October 4th, 2017 and times prior thereto:  Employees worked with and around corrosive chemicals including, but not limited to, Nexeo Surfactant NP-9 (polyethylene glycol).  The emergency eyewash/shower station (plumbed) in the eyewash/shower station area was not in compliance with ANSI/ISEA 358.1-2014 for Emergency Eyewash and Shower Equipment in that:   1) Weekly visual inspections of the eyewash/shower station were not conducted. The emergency eyewash/shower unit were not activated weekly to verify proper operation.   2) The path of access to the eyewash/shower station was obstructed by material limiting the employees access to the eyewash/shower.       b.)    Right Pointe, LLC, Emergency eye wash station-Product testing area, on or about October 4th, 2017 and times prior thereto:  Employees worked with and around corrosive chemicals including, but not limited to, Nexeo Surfactant NP-9 (polyethylene glycol).  The Speakman brand emergency eyewash station (Non-Pressurized Self-Contained Eyewash) in the product testing area was not in compliance with ANSI/ISEA 358.1-2014 for Emergency Eyewash and Shower Equipment in that:   1) Weekly visual inspections of the eyewash station were not conducted. The emergency eyewash unit was not visually inspected weekly to verify the flushing fluid does not need to be changed or supplemented.   2) Access to the eyewash unit was obstructed by items including, but not limited to a table.
