29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Warehouse, on or about 10/4/17:  Pick Mods 4-10, 4-20, 4-30, 5-10, 5-20 and 5-30, ingoing nip points on two runs of conveyor belts with metal lacings, measuring 4 inches wide, at each of the pick mod levels, were not guarded.    Abatement certification must be submitted for this item.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a)  Salvage Area, on or about 10/4/17:  A relocatable power tap (RPT) providing current to two extension cords to charge the Columbia chariot and a JLG lift, were not in accordance with the listing and labeling and were being used as permanent wiring.    b)  Bottle Room, upstairs, on or about 10/4/17:  A relocatable power tap (RPT) providing current to a Dayton pedestal fan was not in accordance with the listing and labeling and was being used as permanent wiring.
