On or about October 2, 2017, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

On or about October 2, 2017, at the above addressed jobsite, employees performing residential roofing operations, nine (9) feet, five (5) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
