Pelsue: On or about October 4, 2017, and at times prior, employees were exposed to crushing and amputation hazards due to lack of adequate guarding for ingoing nip points on the Pexto roller in the metal fabrication area.    (b) T.A. Pelsue: On or about October 4, 2017, and at times prior, employees were exposed to amputation and laceration hazards due to lack of adequate point of operation guarding on the Amada RG-100 and RG-80 press brakes in the metal fabrication area.    (c) T.A. Pelsue: On or about October 4, 2017, and at times prior, employees were exposed to impact or crushing hazards due to lack of adequate guarding for moving parts on the Amada EMK 3510NT and PEGA-345 turret punches in the metal fabrication area.    Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
