OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to an amputation hazard: Technetics 4-Post Press, 10 Old Webster Road, Oxford, MA: On and before September 30, 2017, while putting dies into the universal die holder employees were permitted to use wood blocks to support the top die. The wood block used on September 30 was comprised of two wooden blocks set end to end and taped to make one piece. When the block failed it exposed employees to an amputation hazard.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Technetics, main aisle on the way to the 4 post press, 10 Old Webster Rd, Oxford, Massachusetts; On and before October 5, 2017 the tongue guard on the right wheel was adjusted to approximately three quarters of an inch from the wheel instead of one fourth inch exposing employees to struck by hazards.
