29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about October 5, 2017, in the weighing station room, the employer did not guard the two ingoing nip points on the conveyor line. The employees will take milk off the conveyor line to weigh it. The employees are exposed to the hazard of being caught-in the rotating plastic belts.    b) On or about October 5, 2017, in the filling station room, the employer did not guard the ingoing nip point on the conveyor line. The employees will walk near the line and inspect the empty milk bottles. The employees are exposed to the hazard of being caught-in the rotating plastic belt.   c) On or about October 5, 2017, in the packaging room, the employer did not guard the ingoing nip point on the conveyor line. The employees work in this  area and walk the line to inspect the boxes. The employees are exposed to the hazard of being caught-in the rotating plastic belt.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):       a) On or about October 5, 2017,  in the packaging room, the box erector was not guarded. The employees place boxes on box erector chute. The box erector grabs the boxes and opens them up. The employees are exposed to the activation of the suction cups/arms of the box erector. The employees are exposed to a struck- by hazard.    b) On or about October 5, 2017, in the filler room, there are two areas on this machine that rotate when filling the milk bottles.  Employees exposed to struck-by hazard from the rotating milk dispensers.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:    On or about October 5, 2017, in the bottling room,  here were two rotating protruding shafts on the conveyor line. The shafts projected out more than one half the diameter of the shaft.  The employees will walk the conveyor line to inspect it. Employees exposed to the hazard of being caught-in the rotating shaft. The rotating shafts protruded out approximately three quarters of an inch.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:  On or about October 4, 2017, in the packaging room, the chain and sprocket were not guarded on the out-going side of box erector machine. Employees work and walk along the line to ensure the boxes move on the conveyor. The employees exposed to a caught-in hazard from the rotating chain and sprocket.
