29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): JOBSITE: The employee spraying insulation while standing on a 2 X 6 wooden plank was not protected from the hazard of falling between 9 feet and 13 feet to the lower level by the use of guardrails, personal fall arrest system or other equivalent means. On or about 9/29/17, the employee standing on the above wooden plank to perform the tasks tripped and fell between 9 feet and 13 feet to the lower level and sustained severe injury to the head and fractured the vertebrae from the incident.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: JOBSITE: The employees exposed to fall hazards of up to 13 feet above lower levels while spraying insulation were not trained in fall hazards and the procedures to be followed to minimize such hazards.

29 CFR 1910.134(f)(2): Fit testing. The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. JOBSITE: The employees, wearing tight fitting full face mask respirators while performing spraying insulation activities had not received fit testing prior to initial use of the respirator and at least on an annual basis as required.
