29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: a) Production Area: An employee, Machine Operator, operating the NP120 and cold headers was exposed to continuous noise at 66.18% of the permissible exposure level (PEL) of 90 dBA or time-weighted average of 87.0 dBA. This exposure was 1.98 times the reduced action level of 33.5% dose or 82.1 dba allowed for a 12 hour work exposure period. This exposure was observed over a 692 minute sampling period on October 16, 2017. The employer did not develop or implement a monitoring program, on or about October 16, 2017. No abatement certification or documentation required.

The employer did not institute a training program and ensure employee participation in the program: a) Production Area: An employee, Machine Operator, operating the NP120 and cold headers was exposed to continuous noise above the permissible exposure limit for a 12 hour shift and the employer did not train the employee on the requirements of this section, on or about October 16, 2017. (See citation1 item 1a for full description) Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement. ABATEMENT NOTE: Employees shall be informed of the following: 1. The effects of noise on hearing. 2. The purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, and instructions on selection, fitting, use, and care. 3. The purpose of audiometric testing and an explanation of the test procedures.
