29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: Custom Staffing, Inc. at the worksite located at 305 Walnut St., Bloomdale, Ohio: On or about September 13, 2017, the employer did not ensure that employees who perform maintenance and servicing such as, but not limited to, cleaning and changing the blade on the Gala pelletizer were trained in the skills required for the safe application, usage, and removal of energy controls. Employees that changed the pelletizer's blades were not trained to control hazardous energy sources of the machine.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Custom Staffing, Inc. at the worksite located at 305 Walnut St. in Bloomdale, Ohio: On or about September 13, 2017, employees operated a powered industrial vehicle(s) to lift and maneuver materials such as, but not limited to, boxes and bags of plastic. The employer did not ensure that employees successfully completed a forklift operator training program containing elements in 29 CFR 1910.178(l) before being permitted to operate the forklift.
