29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person. Location: 27 Prospect Street, Melrose, MA 02176. On or about October 10, 2017, an employee was working on a scaffold at a height greater than nine feet above lower level and it partially collapsed causing the employee top fall to lower level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. Location 27 Prospect Street, Melrose, MA 02176. On or about October 10, 2017, six employees were exposed to four fall hazards that reached heights greater than sixteen feet above lower level while working on second floor decking. First instance: Six Employees were raising a gable wall on the second floor decking at a height greater than nine feet above lower level with no fall protection system to prevent them from falling. One of the employees stepped off the second floor decking onto staging next to second floor decking. Staging collapsed and employee fell to lower level. Second instance: Wall opening located in the rear section of the house that had an outside measurement of greater than eleven to greater than sixteen feet above lower level. Inside fall height was greater than fifteen feet above lower level. Third instance: Concrete deck that is greater than nine feet above lower level that employees used to retrieve wood used for framing. Fourth instance: Unprotected window opening located on the first floor, west side of the house had a fall hazard that was greater than twelve feet above lower level. There was a bundle of nails for a pneumatic nail gun on the bottom sill that was only twenty one inches in height on the inside of house.
