On or about October 4, 2017, the employer did not implement a continuing, effective hearing conservation program for an employee (operator), who was exposed to noise at an eight-hour time weighted average of 90.1 dba which is 110.1% greater than the action level of 85 dBA during the 517 minutes sampling period on October 4, 2017.
