29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about October 5, 2017, employees performing residential roofing tear off operations were exposed to struck by hazards when employees worked on two different levels when the employer failed to mandate the use of hard hats. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about October 5, 2017, employees performing residential roofing operations were exposed to a 10.1 foot fall when the employer failed to ensure that fall protection was utilized. No abatement certification or documentation is required of this item.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. On or about October 5, 2017, employees performing tear off operations on the roof at a multi-family property were exposed to a 10.1 foot fall when the employer failed to ensure that a ladder was provided to access the front porch roof. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
