At this establishment, on or about October 6, 2017, the employer, Unique Employment I, Ltd., failed to provide copies of the injury and illness records (OSHA Forms 300 and 300-A) for calendar years 2013, 2014, 2015 and 2016 to an authorized government representative (OSHA compliance officer) within four business hours.
