29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems were rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level: a. On October 2, 2017 employees working at Gulf Power on the south side inside of a cooling tower at the Chris Plant installing sprinklers to the rafters did not ensure his lanyard prevented him from falling more than 6 feet from his working surface, exposing employees to a 30 feet fall hazard.
