29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: On or about October 5, 2017, employee was not provided with special hand tools for removing scrap meat and material from between the blade and the back plate on the band saw. Employee was exposed to the hazard of making contact with the point of operation on the band saw.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about October 5, 2017, employees were operating a bandsaw connected to a pendant electrical cord. The pendant cord had the outer insulation pulled back at the connector on the ceiling, exposing the inner electrical wires to damage. Employees were exposed to a hazard of electrical shock.
