29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift:  a. Front of Building - On or about October 5, 2017 an employee did not connect his lanyard to the basket or boom while working from a Genie S-60X aerial lift basket.  And also, the employee did not buckle the harness leg straps.
