(a) Green Sativa at 3990 Grape Street, Denver CO 80207: On and preceding 10/6/2017, the employer did not provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting. This condition exposed employees to safety hazards.

Employees worked in a cultivation facility that used carbon dioxide for enrichment of plant growth. This condition exposed employees to chemical hazards associated with use of chemicals such as but not limited to carbon dioxide, pesticides and nutrients.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. Employees worked in a cultivation facility.  The employees were required to wear 1/2 mask tight fitting air purifying respirators with organic vapor cartridges while applying pesticides.  Employees hung their respirators in the bathroom.  This condition potentially exposed the employees to respiratory hazards.
