29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (A) On or about October 3, 2017, there were no frequent and regular inspections being performed by a competent person to ensure the jobsite, materials and equipment such as fall protection systems and/or devices were being maintained and utilized in a safe condition.   (B) On October 6, 2017, there were no frequent and regular inspections being performed by a competent person to ensure the jobsite, materials and equipment such as fall protection systems and/or devices were being maintained and utilized in a safe condition.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  On or about October 3, 2017, located on the south end of a commercial roof structure, an employee was performing roofing work (obtaining core samples) on a low-slope roof with unprotected sides or edges at a fall height greater than 35 feet above the ground below that had not been instructed on the applicable procedures and/or regulations to minimize those hazards while performing this work.

29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.  The employer did not have written certification of fall protection training for an employee observed performing roofing work on a commercial roof structure while not utilizing a fall protection system and exposed to potential fall heights in excess of 35 feet.
