29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:           a) On or about October 10, 2017, employees were exposed to eye injury while using electric saws to cut plywood without the benefit of eye protection.          In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):             a) On or about October 10, 2017, employees were exposed to the hazard of falling while performing carpentry work from pitched roof surfaces having ground to eave heights of approximately 19 and 21 feet.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:             a) On or about October 10, 2017, employees were exposed to the hazard of falling when required to climb portable ladders while carrying plywood sheathing materials to a roof deck landing surface.           In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
