This was most recently documented at 12738 S 38th St, Bellevue Nebraska on or about October 11, 2017 when employees were observed installing shingles to a residential structure while working greater than 6 feet above the ground without fall protection.

This was most recently documented at 12738 S 38th St, Bellevue, Nebraska on or about October 11, 2017 when employees were observed using a ladder which was installed with its lower support rails on the peak of a roof.

This was most recently documented at 12738 S 38th St, Bellevue, Nebraska on or about October 11, 2017 when employees were observed using a ladder to access a roof and one of its rungs was broken.
