29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: Employees were exposed to respiratory hazards. Employees were required to wear either an N95 Particulate Respirator, or a half faced respirator with cartridges. There was no written program with specific procedures regarding their use including selection of respirator, medical evaluation, fit testing, and training.

29 CFR 1910.178(q)(5): Parts of powered industrial trucks requiring replacement were not replaced with parts equivalent in safety with those used in the original design. Employees were exposed to struck by hazards in the warehouse as the Nissan forklift had a horn that wasn't operating. The Nissan forklift also had half the seatbelt missing. The forklift had not been taken out of service until it was in safe operating condition.

Guarding was not provided for the Rotolox Rotary Valve. There was no point of operation guarding for the Rotolox Rotary Valve.
