29 CFR 1910.217(c)(2)(i)(d): It shall utilize fasteners not readily removable by operator, so as to minimize the possibility of misuse or removal of essential parts; a) The modified full revolution Bliss press guarding doors were not equipped with fasteners that did not allow employees to open the doors without the use of a tool, in that the fasteners being used were wing nuts and on September 9, 2017, the operator opened the fasteners to gain access to a part stuck in the press.

29 CFR 1910.217(c)(2)(iv): An interlocked press barrier guard shall be attached to the press frame or bolster and shall be interlocked with the press clutch control so that the clutch cannot be activated unless the guard itself, or the hinged or movable sections of the guard are in position to conform to the requirements of Table O-10. a) On the modified full revolution Bliss press, the employer did not install interlocks switches on the gate doors to prevent movement of the clutch control when the gate door struck the clutch mechanism causing the clutch to activate the ram.

29 CFR 1910.217(e)(1)(i)(A): Conduct periodic and regular inspections of each power press to ensure that all of its parts, auxiliary equipment, and safeguards, including the clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in a safe operating condition and adjustment; a) The employer did not implement inspection procedures for the modified Bliss Full Revolution presses in the sign department to ensure that operation of components were within the manufactures specification and the modifications including but not limited to the addition of the air dump valve on the flywheel, the addition of the red stop button on the control, the addition of the guard around the ram that had an opening of 13 inches by 16 inches to allows the operator to reach inside of the press and the interlocked ram blocks.

It shall be the responsibility of the employer to insure the original and continuing competence of personnel caring for, inspecting, and maintaining power presses. a) The employer did not train maintenance employee(s) on how to inspect the Bliss Mechanical Power Presses that had been modified to ensure that components including but not limited to the additional clutch brakes and air dump valves are operational in accordance with manufactures preventive maintenance schedule and requirement.

29 CFR 1910.217(f)(2): Instruction to operators. The employer shall train and instruct the operator in the safe method of work before starting work on any operation covered by this section. The employer shall insure by adequate supervision that correct operating procedures are being followed. a) The employer did not train the operators on the safe methods of operating the modified Bliss Full Revolution presses located in the sign department nor did the employer ensure that adequate supervision on correct operating procedures were being followed in that, operators were not trained on the purpose of the dump valve, the red button on the control panel, the purpose of the opening within the gate around the ram, the sizing of the tooling used to remove stuck parts and the difference between a full revolution and a part revolution press.
