OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and struck-by hazards: On 10/11/2017, at the above addressed work site, the truck mounted crane's automatic rotation lock out system had been modified (hotwired) providing a constant 12-Volts to the turn motor solenoid, which allowed the boom to continue to operate when a pick is being conducted with the outriggers not fully extended. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
